Student's Grade Level for the upcoming year:*The Student Grade is only for overall reporting purposes for the state. The state does not require each student's grade level, but does require a headcount of how many students a private school has in each grade. If your child spans more than one grade, just choose the lower grade.

Scholarships

Until HB1 passed in 2023, it didn't make any difference to CHA whether your students have a scholarship or not. As of June 1st, 2023, there are some changes in the law that cause us to need to know if you have one of these scholarships AND want to use FLVS:

  • Family Empowerment Scholarship for Unique Abilities
  • Family Empowerment Scholarship for Educational Options
  • Florida Tax Credit Scholarship

If your child does not have a scholarship, please choose 'none' in the choices below. If your child does have a scholarship, please let me know which one so that I can know better how to serve you.

Attendance Entry

Please submit the total number of days (180 or more) attended for this student in the space below. The days attended are counted from the date of enrollment last year until the renewal date this year. You may still turn in the separate attendance form if you wish for me to put it into your student's file, but that is no longer required.

You are not required to school every month, or even during the normal 'school year'; however, you must show at least 180 days of attendance over the course of a 365 day year.

(Note) Florida Attendance Requirements: Florida Statutes require that all private, parochial, or public school students who are receiving government funding or scholarships attend school for a minimum of 180 days per year and for the hours (or weekly equivalent) below:

  • Kindergarten: 3 hours per day/15 hours per week
  • 1st-3rd: 4 hours per day/20 hours per week
  • 4th-12th: 5 hours per day/ 25 hours per week.  

Please think of this as a guideline only - education can happen in many different ways and does not always happen "in the books".
