Welcome Back! No matter what took you away, I'm so glad that you have come back to CHA. If you have a returning student, more than likely, I have the enrollment documents in your student's folder from your first enrollment. If you've been away for a long time or you have a new student to enroll, I will definitely need an updated immunization form (or waiver) and health form (or waiver) - and the birth certificate for the new student. I have a place for you attach the documents at the bottom of this form, but you can wait and send them to me via email, mail, or fax (239-294-3558) if you would like. Contact me at [email protected] if you need my address for mailing or have questions about whether I need the updated documents.

Scholarships

Until HB1 passed in 2023, it didn't make any difference to CHA whether your students have a scholarship or not. As of June 1st, 2023, there are some changes in the law that cause us to need to know if you have one of these scholarships AND want to use FLVS:
